Surgery Scheduling Plan
To efficiently schedule your surgery, we have developed a structured plan to ensure a smooth and seamless process as following 3 steps:
Step 1: Communication and Required Information
Email correspondence is the most reliable method for exchanging information and documenting your requests, allowing us to offer tailored advice for your surgery. Accurate completion of the "Personal Health Information Form" is crucial, as undisclosed conditions can lead to complications. Your cooperation is essential for a safe surgical experience. If you have any questions, our team is here to assist via email.
In email correspondence stage, the following steps will be required:
1. Complete “Personal Health Information Form”: Provide a comprehensive overview of your health conditions.
2. Submitting Photos for Professional Evaluation: Please be aware that submitting photos is necessary for some procedures but not all. We will inform you if this step is required for your specific procedure.
Photos Submission for Professional Evalution
If you would like to receive a professional evaluation and feedback from our medical personnels and surgeons, please follow the requirements outlined below.
For facial cosmetic or facial feminization surgery (FFS), you are required to provide:

Step 2: Proposing a Surgery Date
When you're ready to schedule your surgery, please inform us of your preferred month. LMS will then propose available dates for your procedure based on your surgeon’s schedule.
After we reach a mutual agreement on the date, please follow these steps to finalize it:
1. Confirm the Proposed Date: Confirm your agreement within 7 days after receiving the proposed dates from LMS. If we do not receive your confirmation within this period, the date will be made available to other patients.
2. Submit a Deposit: Once the date is confirmed, send a 20% deposit via international wire transfer within 7 days. Please ensure payment is made within the specified time to avoid the need for rescheduling if the agreed date becomes unavailable.
Please ensure timely completion of these steps to secure your surgery date.
Step 3: Pre-Surgery Laboratory Test Requirements
Complete all laboratory tests 4-6 weeks before your scheduled surgery date. Reports older than 6 weeks will not be accepted.
Send your x-ray images, reports, and any required referral letters or medical clearances in PDF or JPEG format as soon as you have them.
The medical team needs to review these reports at least 3 weeks before your surgery date to ensure we have up-to-date information on your health. The standard laboratory tests required are:

Pre-procedure Instruction
Laboratory Test
Patients must complete all lab tests between the 4th and 6th week prior to your surgery date. Our medical team will not accept laboratory reports that are older than 6 weeks before surgery.
Once you have received your lab reports, x-ray images, and referral letter/medical clearance (if needed), please send them to our office in JPEG or PDF format as soon as possible.
It is essential that our medical team reviews all required laboratory reports at least 3 weeks before your surgery date.

Booking your flight information
Flight Booking
We do not handle flight bookings, so you will need to arrange your own travel. Feel free to choose any airline that offers the best price for you, and you can schedule your arrival and departure from Bangkok at any time that suits you. Once you have your complete flight itinerary, please send us a scanned copy or soft copy at least three weeks in advance so we can arrange your transportation in advance.
Rest assured; we will ensure that everything is managed smoothly upon your arrival in Thailand. We are committed to providing you with the safest and most convenient transportation service.
Updating Flight Information After any Occurred Changes
To avoid any typographical errors, please do not manually type your new flight details. Instead, provide us with an exact copy of the flight information received from the airline. You can either take a screenshot or forward the complete flight details to our email address. Please note, we will not arrange transportation from the airport to the hotel without receiving the updated full flight schedule directly from your airline.
Visa and Travel Information for Surgery
To ensure a seamless arrival and stay in Thailand, it's important to verify whether you need a visa before your visit. Certain nationalities are eligible for visa exemption or visa on arrival, while others must secure a visa from a Thai embassy or consulate before traveling.

Types of Visas
1. Visa Exemption:
Some nationalities are eligible for a visa exemption, allowing a stay of up to 30 days in Thailand. You can check the list of eligible countries on our website or the Thailand Ministry of Foreign Affairs website. If your country is part of the Visa Exemption Program, you do not need to visit the Thai consulate or embassy. Upon arrival at Bangkok International Airport (BKK/DMK), you will receive a passport stamp for a 30-day stay. If you need a longer stay, you can extend your visa exemption for seven additional days at a Thai immigration office for 1,900 Baht.
2. Tourist Visa:
A tourist visa generally allows a stay of 60 days in Thailand, ideal for those who need to stay longer than a month. If you are not eligible for the Visa Exemption Program, you must apply for a Tourist Visa at the Royal Thai Embassy or Consulate-General in your country. Alternatively, you can apply for a Medical Tourist Visa (MT), which also permits a 60-day stay. To apply, contact us at least three months before your surgery date to request a Letter of Appointment and a copy of the hospital registration certificate, which are required for your visa application.

3. Visa on Arrival:
The Visa on Arrival permits a 15-day stay and is available to nationals of specific countries not included in the Visa Exemption Program. However, it is not recommended for patients undergoing extensive surgeries like complete facial feminization or gender-affirming surgery due to the short recovery period. For those needing a longer stay or a different visa type, apply at the Office of Immigration Bureau in Bangkok. Note that visa extensions and modifications are at the discretion of the immigration officer.
Visa Extension Assistance
If you require an extended stay in Thailand and cannot manage the visa extension process yourself, we can assist for an additional fee of 8,000 THB, which includes the visa fee. Ensure the application is submitted before your current visa or 5 business days before the permit expires.
Additional Information
• Check first if you qualify for a visa exemption, which allows a 30-day stay in Thailand. If the visa exemption does not apply to your nationality, you can apply for a tourist visa at a Thai embassy or Consulate-General in your country, allowing up to a 60-day stay.
• If your recovery is slower than expected and you need to extend your stay, you can visit the immigration office (1 or 2 days before your 30-day stay expires) to apply for a visa extension. The immigration office allows only a 7-day extension per application, and there is a cost of 1,900 for the visa extension.

Requirement
for SRS
Requirement for SRS
As per the current regulations in Thailand for Sex Reassignment Surgery (SRS), you need to submit two referral letters from qualified mental health professionals. These letters should be directly addressed to LMS surgeon who will perform your SRS surgery.

The first referral letter must be authored by either a psychiatrist or a licensed psychologist.
-
The second referral letter can be written by a psychiatrist, licensed psychologist, sexologist, or gender therapist.
Submission Guidelines and Criteria for Referral Letters:
1. Diagnosis Confirmation:
-
The letters must confirm your diagnosis of Gender Identity Disorder (Gender Dysphoria).
-
The letters should endorse your suitability for sex reassignment surgery, stating that SRS is the recommended treatment in your case.
-
Ideally, the referral should confirm that you have been undertaking a course of female hormone replacement therapy (HRT) for at least six months unless there are medical reasons advising against it.
2. Signature Requirements:
-
The printed recommendation letters must contain the original handwritten signatures of your specialists. Electronic signatures are not accepted.
3. Submission Process:
-
Send scanned copies of the original SRS letters with handwritten signatures via email (in PDF or JPEG format) at least two months before your surgery date for our team’s review and approval.

To facilitate the approval process, please include a web link to the credentials of your specialists.
4. Consultation Day Requirements:
-
Bring the original letters to the LMS surgical team on your consultation day in Bangkok.
Additional Notes: We do not have a specific form or template for the SRS recommendation letters, as each specialist has their own style. We accept differences in their style or type of composition, provided the essential criteria are met.
Consultation with Thai Psychiatrists in Bangkok
We recognize that you may not have received any counseling or psychiatric evaluation during your transition and might be facing challenges in finding a reputable mental health professional or gender therapist in your area.
Should you be unable to obtain the necessary referral letters in your home country, an alternative is to consult with two psychiatrists in Thailand.

You will undergo a one-time evaluation with each psychiatrist. Please be aware that these consultations do not guarantee surgery approval. It is essential for you to demonstrate to the psychiatrists that your gender identity is female and that you are certain about proceeding with SRS to achieve alignment between your mind and body.

There is a fee associated with these consultations, which will cover the cost of both evaluations.

If you are seeking to consult with psychiatrists in Thailand, it is crucial to arrive earlier than your expected surgery date. This process must be completed before you can proceed with SRS surgery.
By adhering to these guidelines and fulfilling the requirements, you can facilitate a smoother approval process for your SRS and ensure a successful consultation with our team in Bangkok.

Deposit and Cancellation Policy
Deposit, Reschedule, and Cancellation Policy
This policy outlines the terms and conditions agreed upon between the patient and LMS regarding the procedure and its pricing.
Deposit Policy
Once the patient and LMS agree on the procedure and pricing, the following deposit terms apply:

First Deposit: A deposit of 20% of the total procedure cost is required within 7 business days of the agreement.
-
Second Deposit: An additional 30% of the total procedure cost must be paid within 30 business days following the payment of the first deposit.
-
Final Deposit: The remaining 50% of the total procedure cost must be paid upon confirmation of the travel date.
Payment must occur after the surgery date has been agreed upon to reserve the surgery date.
Reschedule Policy
We require patients to inform us at least 2 weeks before your surgery date in case of rescheduling. This advance notice is crucial as it allows us to manage scheduling with the surgical team and hospital effectively.
If notice is given less than 2 weeks before the scheduled surgery date, a rescheduling fee of 10% of the total surgery cost will be applied.
This fee compensates for the administrative and operational responsibilities LMS has towards the doctor team and hospital. Your understanding and cooperation in adhering to this policy are greatly appreciated, as they help us provide optimal care and service for all our patients.

First Reschedule: Patients can reschedule their procedure date up to 4 months from the original date without any additional charges.

Second Reschedule: If a second reschedule is required, an additional fee of 10% of the agreed procedure cost will be charged.
Cancellation Policy

Refund: In the event of a cancellation, patients are eligible for a cash refund of 70% of the amount paid.

Banking Transfer Fee: Please note that any banking transfer fees will be the patient’s responsibility. LMS will deduct these fees from the refund amount before processing it.
This policy ensures that both the patient and the hospital have a clear understanding of the financial commitments and flexibility regarding the scheduling and cancellation of medical procedures.
